Terence H. (Terry) Forde Jr. of Chappaquiddick and formerly of Brookline died peacefully on Dec. 12 with his family at his side. He was 86.

He was a proud graduate of Northeastern University. He worked at the Massachusetts General Hospital’s emergency room laboratory, was clinical director of laboratories and director of the Boston Children’s Hospital admitting department.

Terry was instrumental in working with the Lawrence Neighborhood Association to establish the Ronald McDonald House and the expansion of the Lawrence School playground. He later served as honorary chairman for the Brookline Park Commission. He worked as a visting nurse aide on Martha’s Vineyard for 20 years and served as the president of the Chappaquiddick Island Association.

He was the loving father of Jonathan D. Forde of Brookline, Colleen M. Forde of York, Me. and Michael T. Forde and Tina Gee of West Roxbury; former husband of Mary C. Forde of Wells, Me.; dear brother of Donald P. Forde and Anne Forde of Needham and the late George T. Forde and Marilyn Forde-Romeo, Kevin J. Forde and Diane Forde and Doris Rheaume. He is also survived by his cherished grandchildren Michele (Mikey) Stivaletta, Alaina Stivaletta, Michaela Forde and Devon Forde; and 20 nieces and nephews.

Relatives and family are invited to visiting hours that will be held at the Bell-O’Dea Funeral Home in Brookline on Friday, Dec. 16 from 1 to 3 p.m., with some words of remembrance by family at 3 p.m. Interment will be held privately at a later date.
